How to enroll for online access: . If you receive a status report on a pending case, write the answers to the questions asked on the status sheet you received and fax it back to 916-676-1444, attn: Impaired Risk.  Keeping all information on the same sheet of paper helps to avoid confusion at the home office.

Commission Checks You must have a balance of $75.00 to receive a commissions check Commission Information Commission checks are mailed the 15th and end of every month.  Direct Deposit is not available. Annualization is not available.
